Our practice utilizes a software called PT Everywhere to schedule, invoice, keep records, communicate any sensitive information that needs to be HIPAA compliant, and even record exercise programs for the people in our care. You will be directed to this website when scheduling. Please create an account in order to book your appointments. You may choose to download the PT Everywhere App as well on your smartphone.

We pride ourselves in providing the best individualized care to each client regardless of insurance coverage. Therefore, Dr. Elizabeth Guarin is an out-of-network/cash-based provider. Many insurance providers offer out-of-network reimbursement. We will provide you with the appropriate billing invoice to submit to your insurance carrier. We recommend using Reimbursify to assist with your reimbursement.

**Physical therapy prescription or Direct Access**: You DO NOT need a prescription from your doctor to see Dr. Guarin.  If your plan of care extends beyond 30 days, you will need a doctor to sign off on your plan of care. - Florida Statute – Chapter 486.021 (11) (a)

Initial Consultation/Evaluation

Your initial consultation and evaluation will include a review of medical and surgical history, symptoms, imaging or medical testing performed, assessment of posture, range of motion, strength assessment, balance testing, and functional testing in order to develop your physical therapy plan of care. For pelvic floor patients, an internal exam is typically performed to assess your pelvic floor.

** Out of respect for your time, and the time of other patients, a valid credit card is required for scheduling an initial evaluation**

Follow Up

Your follow up appointment will include implementation of discussed treatment plan, including manual therapy, therapeutic modalities, therapeutic exercises, neuromuscular re-education, etc.
